Pre-information form for the test

Everyone taking a test must fill in the pre-information form (excluding HUS referrals)

Medical certificate A is required for applying infectious disease allowance from Kela. Select this if you want the certificate for a possible positive PCR or antigen test result. The certificate A costs 40 euros and will be invoiced afterwards if the test result has been positive. The certificate will be sent by mail. If the test is negative, you will not be sent or charged for certificate A, even if you have selected it on the pre-information form.

An Official certificate is the only document that allows you to verify a negative test result when traveling. The requirements of corona test certificates have changed in several countries. Please check the regulations of the country you are travelling to.

If you are eligible for reimbursement by Kela (The Social Insurance Institution of Finland), you will be prompted to pay by debit or credit card at the testing site according to the instructions by Kela. Please remember to bring your medical insurance card issued by Kela! Register at least one hour before arriving at the test site.

PLEASE NOTE! If you already have an referral from HUS, you will not have to fill out the details form or pay online. You can come directly to the testing location of your choice.
